Registering as a patientRegistering as a New Patient

Rowe Avenue Surgery has an open list and welcomes requests for registration from patients living in or moving to the practice area. The practice boundary allows us to register patients from Peacehaven, Telscombe Cliffs and East Saltdean. New patients must live within these boundaries to be able to register with us. We will need to seer proof of residence that you live within the surgery boundary.

All patients wishing to join the practice should come into reception and make an appointment for a new patient medical check. Please note that no discrimination will be made in regard to race, gender, social class, age, religion, sexual orientation, appearance or on grounds of disability or medical condition.

If you live within the practice area and wish to register, please complete either your medical card (if you have one) or print off and complete theĀ New Patient Registration Form and return it in person to the surgery.

You will need to bring two forms of identity. One will need to be photo ID, eg passport or driving licence and the other needs to be a recent utility bill or bank statement confirming your address. When you register you will be asked to complete a short questionnaire. Patients may also be asked to make an appointment with a nurse to discuss any ongoing problems and current medication. Medical notes may take several weeks to be transferred from your previous GP, so we ask you to fill out a registration questionnaire which is vitally important if we are to give the best patient care in the meantime.

Under the GMS Contract, patients are registered with the practice rather than an individual doctor. However, a patient is entitled to express a preference for the doctor they wish to see.

Download the New Patient Questionnaire for adults and submit this with your registration form (PDF file).

Download the New Patient Registration Form for children and submit one copy for each child you wish to register (PDF file).

Change of Personal Details

If you change your name, address or telephone number please remember to inform the surgery. This may be done either by visiting the surgery or by phoning us on 01273 579500 / 579505 or by downloading and returning our change of details form (PDF) which you can post to us, drop in to the surgery or fax to us on 01273 579501.

Practice Boundary - Proposed Changes

The proposed changes to Practice Boundaries and the ability to register with a GP of choice, as announced in the press on 17th September 2009, have not been formally adopted and therefore the Practice Boundary remains the same.

More About Your NHS Number

Each NHS Number is a unique 10-digit number which is printed on your NHS medical card or will from now on be written on any communication you have had from the NHS.
